Eligibility

Available to employees, with optional coverage for spouses and dependent children.

Class I (Single):

Employees only — $50,000 minimum,
increments of $25,000, max $500,000

Class II (Family):

- Spouse only: 50% of employee sum (if no dependent children)
- Spouse + dependent children: 40% spouse, 5% per child (max $25,000 per child)
